HOLD HARMLESS AGREEMENT AND LIABILITY WAIVER The Rackyard Gym LLC 4334 Old County Rd 8, Moose Lake, MN READ CAREFULLY - THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS 1. ASSUMPTION OF RISK AND WAIVER OF LIABILITY I understand and acknowledge that participation in activities at The Rackyard Gym LLC-including but not limited to martial arts, boxing, fitness training, weightlifting, boot camps, athletic performance training, sparring, and any other physical activity-carries inherent risks of injury, illness, or even death. I voluntarily assume all such risks. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, fully release and hold harmless The Rackyard Gym LLC, its owners, employees, volunteers, associates, contractors, trainers, affiliates, participants, and the building owners of 4334 Old County Rd 8, Moose Lake, MN, from any and all claims, demands, actions, or causes of action for injury, disability, death, or loss or damage to person or property arising out of or related to my participation in any activities conducted by or at The Rackyard Gym LLC. This includes, but is not limited to, injuries caused by: - Accidental contact - Equipment failure - Negligence (including alleged negligence) - Sparring or combative training - Environmental conditions - Actions of other participants 2. MEDICAL ACKNOWLEDGMENT I certify that I am in good physical condition and have no known medical conditions that would prevent me from safely participating. I acknowledge that The Rackyard Gym LLC is not responsible for providing medical care, and I agree to be financially responsible for any medical care or transportation required due to injury. 3. MEDIA RELEASE I hereby grant full and unrestricted permission to The Rackyard Gym LLC, its owners, employees, representatives, volunteers, contractors, affiliated media professionals, and assigns to capture, use, reproduce, publish, and distribute photographs, video recordings, audio recordings, or any other media of myself (or my child if under 18) taken during any activities, classes, events, or sessions conducted by or affiliated with The Rackyard Gym LLC. These materials may be used for commercial purposes including marketing, advertising, social media, or any lawful purposes. I waive all rights to compensation, royalties, or approval of the final content and release all parties listed above from any claims related to their use. 4. BINDING AGREEMENT This waiver is binding and applies to all future participation unless revoked in writing. If any portion is deemed invalid, the remainder shall remain in full force and effect. 5. PARENT/GUARDIAN CONSENT FOR MINORS If the participant is under the age of 18, I certify that I am the legal parent or guardian and grant permission for participation. I agree to all terms stated above on behalf of the minor.
